After undergoing the NorthStar process this form must be signed by a Master Mason in Good Standing at Harmonie with the first 1/4th of the fee of $100.00. The next $100.00 due at time of each Degree.

This is for a Regular Master Mason to affiliate his membership to Harmonie. This form is required along with a $100 application fee. Dues for the year affiliated are due in full after being elected to membership. They are not prorated. A Master Mason in Good Standing at Harmonie is required to endorse this application.
